I am the defendant named above. Upon oath or affirmation, and under penalty of perjury, I state
as follows:
1. I understand that the Division of Motor Vehicles will revoke the registration of all vehicles
registered in my name or in my business name. I understand that for every vehicle
registered in my name as co-owner or as co-owner under a business name, the Division of
Motor Vehicles will reissue vehicle registration without my name.
2. Following are all vehicles (including cars, trucks, snow machines, all-terrain vehicles,
trailers, boats, and planes) registered in my name or my business name, as owner or co-
owner, as of the date of this Affidavit (use additional pages if necessary to list all vehicles):

3. I understand this is notice to me of the registration revocation of all vehicles I own. Any
co-owners must take the vehicle title to the Division of Motor Vehicles to have my name
removed from the record before registration will be restored. (Aircraft revocation will be by
the FAA.)
