The social security number on the 4506-T form must match the one on the fax cover sheet
• The 4506-T form has to be signed and dated within 59 days of your request
• If you are requesting the 1040 form for borrowers who did not file joint returns, attach a 4506-T
form for each one
• 1120 form requests, the 4506-T form has to be signed by one of the officers of the company
(include title next to signature)
• Remember to provide us with the business name and Tax ID #
