Please have this form completed and signed by your local jurisdiction authority, which is the city your
business is physically located in. To obtain contact information for your local jurisdiction, please visit
mrsc.org/cityprofiles/citylist.aspx. Each For Hire vehicle certificate is $55.00. If you have NOT paid a For Hire
Licensing fee to your local jurisdiction, you must send an additional $110.00 with this addendum for a state
For Hire permit.
