ACCEPTABLE DOCUMENTS

ONLY NEW YORK STATE RESIDENTS WHO ARE ALSO U.S. CITIZENS CAN APPLY FOR AN ENHANCED DRIVER LICENSE OR AN ENHANCED

NON-DRIVER ID CARD.

When you first apply for an

Enhanced Driver License (EDL), or if you currently have a NYS photo document, you must provide proof of your Social Security Number

(SSN), identity, U.S. citizenship, and New York State residence as described below:

1. Proof of Social Security Number (SSN):

u You must show your original Social Security Card OR

u If you have a valid NYS driver license, or a non-driver photo ID card that was issued after 12/10/2002, you can show one of the following documents instead of

your Social Security Card:

s Your federal or NYS income tax form W-2 with your full SSN, OR

s Your federal or NYS income tax earnings statement (form 1098 or 1099) with your full SSN, OR

s A U.S. computer-printed pay stub with your full SSN and your name and address.

2. Proof of Identity:

u You must show your Social Security Card and you must show one or more of the acceptable types of documents that total at least 4 points for proof of name

(refer to the table below)

u You must show one of the documents listed under “Proof of Date of Birth?” in the table below to prove your date of birth.

3. Proof of United States Citizenship: You must show one of the documents listed under “Proof of U.S. Citizenship?” in the table below.

4. Proof of New York State Residence: You must show two of the documents listed under “Proof of NYS Residence?” in the table below.

u Each document must show your residence address. An address with a P.O. box is not acceptable proof of residence.

u You may use proof-of-residence documents that are in your spouse’s name if:

s You and your spouse have the same last name, OR

s You show additional documents that prove your relationship (for example, a marriage certificate).

The table below lists the documents that are acceptable to prove your name, date of birth, U.S. citizenship, and New York State residency.

u You must provide original documents or documents that are certified by the agency that issued them.

u DMV will not accept more than one document of the same type of proof. For example, DMV will accept only one major credit card or one utility bill.

u DMV will not accept documents with any alterations or erasures. DMV will confiscate any document that appears to be fraudulent so the document can be reviewed by

DMV’s Investigation Unit. If you submit a fraudulent document, you could be subject to criminal prosecution.

IMPORTANT: Making a false statement in an application or in any proof or statement in conjunction with it, or deceiving or substituting, or causing another person to

deceive or substitute in connection with such application, is punishable as a Class A misdemeanor pursuant to Section 210.45 of the Penal Law for Non-Driver card applicants,

and is a misdemeanor under Section 392 of the Vehicle and Traffic Law for driver license applicants, and may result in the revocation or suspension of your license.
