The servicer uses this form to document the property inspections it makes to determine the condition of properties owned or securitized by Fannie Mae.

Original, or original plus one.

Printing Instructions

This form must be printed on letter size paper, using portrait format.

Instructions

Although the servicer may use this form to record the results of any property inspection it makes, we only require that it be submitted to us to document an

updated comprehensive property inspection report (which should be included in the property case file that is submitted immediately following, or at least no

later than 15 days after, the foreclosure sale or the date a deed-in-lieu was executed). In this instance, the servicer should retain the copy and submit the

original form to the regional office.
