2015!LETTER!OF!INTENT!INSTRUCTIONS!&!FORM!

The$Children’s$Cardiomyopathy$Foundation$(CCF)’s$Research$Grant$Program!has$a$two =step $application$

process.$A$letter$of$intent$is$required$in$advance$of$submitting$a$full$grant$application.$The$2015$deadline$for$

letters$of$intent$is$Friday,$June$12$by$5:00$p.m.$eastern$standard$time.$

The$following$instructions$are$for$investigators$who$are$interested$in$being$considered$for$CCF’s$Research$

Grant$Program.$Only$investigators$who$have$submitted$a$letter$of$intent$and$have$been$invited$by$CCF$to$

subm it$a$formal$grant$application$w ill$be$considered$for$funding.$$

Letter!of!Intent!Requirements!

The$letter$of$intent$is$designed$to$provide$a$general$overview$of$the$proposed$research$study$with$sufficient$

detail$to$allow$for$the$evaluation$of$the$scientific$merit$and$feasibility$of$the$study.$Two$pages$(letter$of$

inte n t$fo rm$an d $re se ar ch $a b str ac t)$a re $req u ire d$fo r$su bm ission$ an d$the$remaining$pages$(biographical$sketch$

and$publication$list)$are$optional.$$

The$documents$should$be$in$the$following$fo rm a t:$single=sided,$single=space d,$Calibri$12=point$font,$1=inch $

margins$all$around$and$with$page$numbers$at$the$bottom .$The$lette r$o f$in te n t$s h o u ld $inc lu d e $th e$fo llo win g:$

The$provided$cover$sheet$shou ld$be$com pleted $and$signed .$

2. Research!Abstract$(1$pag e)$

The$abstract$should$summarize$the$research$project$and$describe$what$issue(s)$the$study$addresses,$

the$stud y’s$spe cific$aim s,$releva nt$literatu re$or$p rior$rese arch ,$how $the$study$will$be$conducted$and$

evaluated,$and$why$the$work$is$important$to$furthering $kn owledge$abou t$pediatric$cardiomyopathy$

or$improving$the$treatment$of$children$with$the$disease.$

3. Supporting!Documents$$

The$following$supporting$documents$are$optional$but$may$be$subm itted$with$the$letter$of$intent$and$

researc h$ab strac t.$

• Biographical$sketch:$This$should$include$education$and$training$in$c h ro n o lo g ica l$o rd er ,$

positions$and$honors$in$chronological$order,$and$past$and$current$research$support$for$the$

past$three$years.$The$NIH$biosketch$format$is$pref err ed .$A n $exam ple$can$be$downloaded$

• Relevant$publications:$This$sh ould$be$a$short=lis t$o f$peer=rev iew ed $publications$from $th e$p ast$

5$years$that$are$relevant$to$the$proposed$research$top ic.$

A$PDF$copy$of$the$letter$of$intent$and$supporting$documents$should$be $electronically$submitted$(email)$to$

lyue @childrenscardiomyopathy.org$no$later$than$Friday,$June$12,$2015$by$5:00$p.m.$eastern$standard$tim e .$

Each$letter$of$intent$is$carefully$re view e d$by $CCF’s$me d ic a l$a d v isors$a n d $C C F ’s $P re si d e n t$a n d $E x e c u tiv e $

Director$to$determ ine $if$th e$p ro je ct $is$in $lin e$with $CCF’s$rese arch $criteria$and$priorities.$Preference$will$be$

given$to$stu dies$on$primary$cardio myo pathy$and$where$no$other$funding$organization$exists$for$the$form$of$

cardiomyo pathy$being$studied.$Inv es tig at o rs$will$be$notified$by$email$no$later$than$Ju ly$24,$20 1 5 $on$whether$

they$ha ve $bee n$ap pro ve d$to$su bm it$a$fu ll$grant$ap plicatio n.$For$eligible$investigators,$the$submission$

deadline$for$full$grant$ap p lica tions$is$Friday,$Septem ber$4,$2015$by$5:00$p.m.$eastern$standard$time $

All$decisions$made$by$CCF$are$final$and$are$not$subject$to$appeal.$
