The following contract and its terms will set forth an agreement between ML Photo (provider) and

____________________________________________________________________ (client) the parties, for photo booth

servicesoutlinedonthesecondpageofthisagreement.

ThisContractsetsforththefull,writtenintentionofbothpartiesandsupersedesallotherwrittenand/ororalagreements

betweentheparties.

TheServicePeriodwillbethedateandtimeslistedonthesecondpageofthisagreement.MLPhotoagreestohavethe

PhotoBoothsetupatleast15minutesbeforethetimeagreedupon.(Shouldyourequiretheboothtobesetupearlieror

you

’dliketheboothtohavedowntime,a$25/hourdowntimefeewillbecharged.)ThePhotoBoothwillbeoperational

foraminimumof90%ofthetimeset;occasionallyoperationsmaybeinterruptedformaintenanceofthephotobooth.

Inordertoprotectexpensiveequipment,MLPhotoreservestherighttorefuseadmissiontoguestswh

oareunrulyand/or

haveconsumedexcessiveamountsofalcohol.BeverageswillnotbepermittedinoraroundthePhotoBooth.Shouldthe

photo booth be damaged in any way due to unruly guests or negligence on the part of the client or client’s guest

Photoreservestherighttoterminatetheserviceperiodwithoutrefund.

Cost&Payment

TherentalfeeforthePhotoBooth is$100/hour,minimum2hours,within30milesofSteubenville,Ohio.Every30miles

outsideofSteubenville,Ohioisanextra$25/hour.Adepositof$100isdueatth

esigningofthiscontacttoreserveyour

date.The remaining amount is due 7 days before the event.Client will pay a $20 fee for any returned checks.The

depositisfullyrefundablewhencancelledinwritingbymailore‐mailatleast30dayspriortotheeve

nt.Ifweareableto

rebooktheevent,onehalfofthedepositwillberefunded.

ThePhotoBoothisavailableforindooreventsonly

.Clientwillarrangeforanappropriatespaceforthephotobooth‐8’

deepx6’widex8’high‐whichmustbenearaworkingpoweroutletorprovideanextensioncord.

DateChanges&Cancelations

Anyrequest fora datechangemustbe madeinwritingat least10daysinad

vance oftheoriginaleventdate. Changeis

subjecttophotoboothavailabilityandreceiptofanewServiceContract.Ifthereisnoavailabilityforthealternatedate

thedepositwillbesubjecttothetermsaboveandeventcancelled.

pose.Inaddition,clientherebyreleases,dischargesandagreestosaveharmlessMLPhotoandthePhotoBooth,

fromanyliabilitythatmayoccurorbeproducedinthetakingofsaidpictureorinanysubsequentprocessingthereof,as

wellasanypublicationthereof,including,withoutlimitation,anyclaimsforlibelinvasi
