Dear Landlord:
KRS 383.595 requires all landlords to comply with the provisions of the lease/rental
agreement and the requirements of applicable building and housing codes materially
affecting health and safety and to make all repairs and do whatever is necessary to put
and keep the premises in a fit and habitable condition.
The following conditions in my dwelling unit constitute a breach of Section 383.595
and/or our rental agreement:

Under the provisions of KRS 383.625, I may terminate our rental agreement not less than
thirty (30) days after your receipt of this notice if these breaches are not remedied within
fourteen (14) days.
In addition, if the same conditions of which such notice was given recur within six (6)
months, I may terminate our rental agreement upon at least fourteen (14) days written
notice specifying the breach and the date of termination of our rental agreement. Should
our rental agreement be so terminated, I would be entitled to the return of any prepaid
rent.
