DAYCARE CONTRACT AGREEMENT
Effective January 1, 2013 – December 31, 2013
I am a group family daycare provider licensed by Sherburne County according to the State of Minnesota Rules 9502-0315 – 9502-0445 for Family Daycare and Group
Family Daycare. I have been a licensed daycare provider for 18 years. I am licensed for a total oftwelve children; ten of which may be under school age; of that ten,
no more than two shall be infants and toddlers. Of those two, no more than one shall be an infant.
HOURS OF SERVICE
Mybusinesshours are 7:00 a.m. to 5:30 p.m.I have a very casualschedule and donot mind whenyour schedule changes withinmy
business hours. Drop offtime before mybusiness hours or pick up time after these hours must beprearranged inpersonor by a
phonecalland onlyonanoccasionalbasis. Anovertime charge of$1.00 perminute per child willbe assessed starting at 5:30 p.m. if I
wasnot notified you would be late for pick-up.You may use a maximum ofnine hours per day and forty-five hours per week or an
additionalcharge willbe added to the fee.
ADMISSIONS
Whenwe meet toconsider having your child join myfamilydaycare, I’llshare detailswithyou about my dailyschedule,methods of
discipline, plans for naps and meals, weekly projects and organized playtimes. I’ll ask foryour input inregards to your child’s
preferences, personalityand current schedule so thatwe canplanthebest way to integrate your child into the group. Please feelfree
to ask any questions you mayhavein regards to anyaspect ofmydaycare.
I donot discriminate inadmissions based on race, creed, color, religion, sexor nationalorigin. I do, however, recognize thatnot every
child will fit comfortablyinto my child care family.For this reason, everynew child beginsona two week trialbasis. During this
twoweek period, the parents or the provider are allowed to give a one daynotice to terminate care on the basisofnotbeinga “good
match.” After the trial period, a two week notice isrequired for either partyto terminate care. Ifyoudecide not to use that last two
weeksofcare, you are still required to paythe fee.
HOLDING FEE
Thisfee guarantees a spotfor aspecified date and age ofchild.This fee in non-refundable should alternate arrangements be made by
you. The amount isequalto one week’s payrate and will be credited to fees owed the first week you begin daycare.
RECORDS
I am required bySherburne Countyto have certain writteninformationand permissions on file inorder tocarefor your child. I will
provide you withtheappropriate forms and ask that theybe returned on or before the firstday ofcare. Please notifyme ofany
updates as theyoccur. Information regarding yourfamilyis keptprivate except for the reporting I mustdo tomylicensing agent. I
am alsorequired byMinnesota law to report any suspected cases ofchild abuse toSocialServices. Thisincludes physical, emotional
and sexualabuse, as wellas neglect.
VACATIONS
I willtake up to four weeks ofvacation eachyear. I send out e-mails to notifyyou ofmy time offwithplentyof advanced notice.
One week out ofthefour willbe a paid vacation. The paid week willbe the week I take offinJanuary eachyear.Paymentis due the
Fridaybefore myvacation, but a check maybe post-datedand willbe deposited uponmy return.
Occasionally, dayswill come up thatare not on the memo, but you will receive atleasta twoweek notice ofthese single days. I take
closing my daycare veryseriously, because I realize the inconvenience to you ifyou cannot depend ofyour daycare each morning. In
the fifteenyears I’vebeenindaycare, I’ve had two sick days where I had to calltheparents in the morning to notify them thatdaycare
had to be closed forthat day. Whenitis necessaryfor me to close daycare, your weekly rate willbe adjusted accordinglywith
exceptionofthe one paid week of vacation. You are required topayyour normalfeefor your child’s absent days whether it’s
vacation time or sick days.
LEAVE OF ABSENCE
Aleave of absence willbegranted due to a work lay-off,lossofjobor maternityleave. Ifa child isabsentfrom daycare, you canpay
a minimumfee of$75 per week to hold the positionopenfor up tosixweeks peryear.After sixweeks, the ratewill return to the full
amount or I willneed to fill thevacancywith another child.
PAID HOLIDAYS
Thefollowing holidays are considered paid, contracted holidays:

Whenthese holidaysfallon a weekend, I’lltake either the Fridaybefore or Monday after as the paid holiday.
SICK CHILD POLICY
I ensure that allsubstitutecaregivers and myselfwillnot abuse prescription medicationor be under the influence ofany chemicalsthat
would impair our abilityto provide qualitycare.
I willnotifyyou ifyour child becomes illduring the day. This includes, but is not limited to, a fever of101 degreesor more,
vomiting, diarrhea or a rashother than a diaper rash or heat rash. If I’mworried about the child passing their conditionon to the other
children or the child would be more comfortable at home resting, I willnotifyyou.Whenyou begindaycare, I request you provide a
bottle ofpain reliever and a bottle ofcold medicine labeled with your child’s name to be used byyour child as needed. Please donot
bring your child to daycare if theyhave suffered fromany ofthese symptoms within the past 12 hours.
Ifyour child needsto be on a specialdiet, please send those items along withyour child. I maynot have the necessaryitems on hand.
Ifyour child hasbeen exposed to any contagious diseases oris diagnosed withone, please informmewithin 24 hours so I maynotify
the other parents.
Please notifyme immediatelyifyour child has beenexposed to a contagious disease or diagnosed with a serious illnessor medical
condition so that I canalert the other parents.
MEALS:
Your child willbe enrolled in the Child Care FoodProgram. This is a federallyfunded programand insures thatyour child will be
served nutritionally balanced meals.Meals willbeserved at the following times:
Breakfast: 8:30 a.m.Lunch: 12:00 p.m.
A snack willbe served mid-morning and mid-afternoon
Achild willbe expected to eat whatis served for eachmeal.Iftheydislikeanitem,theywillbe encouraged to taste it,but no
alternatives willbe served.Children who choose not toeatamealor snack willneed towaituntilthenext scheduled time.
Ifyour child will be arriving after a meal time has begun, but you still wantyour child to eat here, priornotice isrequired.Ifyour
child comes after a mealtime, it willbeyour responsibilitytomake surethatchild has eatenprior to arriving. Bringing a mealfroma
restaurantand having the child eat here causes hard feelingsfrom the rest ofthe children and is therefore, notallowed.
Infantformula: Ifyou have aninfantusing formula, you have the choice ofproviding theformula yourselfor having me provide the
formula and having the infanton the countyfood program. IfI amtoprovide the formula, itmust be within the average priced
formula. Ifyou choose a premium-priced formula,you mustprovide ityourself.
NAPS:
Children under the age offour years old are required to lie downin the afternoon. Naptime is from1:00 p.m.–3:00 p.m. Infants will
sleep inanapproved porta-cribinanupstairs bedroom.Toddlers and preschoolers sleep inbunk beds in the downstairs bedrooms. At
the age offour, ifyou and I agree theyare ready to give up naptime, theycan take quiet time in the playroom watching a movie while
the other childrennap.
PERSONAL ITEMS
Please provide me with the following items:
1.Achangeofclothing, labeled with the child’s name, to beused in case of an accident.
2.Abottle ofpain reliever and a bottle of cold medicine.
3.Diapers for those childrennotpotty trained. (Wipes willbe provided.)
TOYS FROMHOME
Please do not letyour child(ren) bring their toys fromhome. When a new toycomes into daycare and allthechildrenwanta turnwith
that toy, itis extremelyhard toshare withallthe other childrennomatter how good a child is atsharing.Iftheyhavea blanket to
sleep with, that would be an acceptable itemtobring with them,butI’d rather have a blanket here for themso there isnoproblem
remembering itmorning and evening.
TOILET TRAINING:
I feelmostchildrenare readyto begintoilet training when theyarebetweentwoand three years ofage. Thisis something we should
discuss betweenus, since consistencybetween ourhomes willbe so important. Dressing your child in one-piece outfits such as
overalls or using pants withdifficultsnaps will cause frustration. Please take this into considerationeachmorning during the potty
training process. Be sure to provide me with severalextra outfits incaseof accidents.
DISCIPLINE:
I usepositive discipline techniques suchas making myexpectations clear, reminders, and redirectionbefore resorting to time-outs and
taking privileges away.Physicaland emotionalpunishments willnotbe used in my daycare. You willbe informed ifa problem
persists or if I need cooperationfromhome so that we canbe more consistentin whatis expected. Routine problemswillbe handled
without making a big dealofthem.
SMOKING:
As provided in the licensing Rule, smoking isprohibited in myhouse during daycare hours. Please donot smoke onmy property or
extinguishcigarettes inmyyard or driveway.
PETS
We have a catnamed Peanut. Heis declawed in the front and has a currentrabies vaccination. The litterboxis keptin the garage out
ofaccess to the children. He isbrushed regularlytokeephair to a minimum.
FIRE ANDSTORMDRILLS
Afire and escape planhasbeen drawnand is posted bythe front entrance. Fire and stormdrills are practiced monthly to teachthe
children how to dealwith an emergencysituation.Myhusband and I are both trained in basic first aid and infant/child CPR
TRANSPORTING
I willnot, under anycircumstances, transport your child in a vehicle. I do not have properchild restraints inmy vehicle and I do not
have the Child Passenger Safety Training required by the countyifI were to transport children who attend my daycare.
LIABILITY INSURANCE
I have liability coverage onmyhomeowners insurance policy tocover my daycare business.
GRIEVANCE POLICY
Ifyou are unsatisfied withmyservices or policy, please feelfree to communicate these concernswith me atany time.
FEES(See attached rate card)
Payment isdue everyMonday to cover the upcoming week. Alate charge of $5.00 per day willbe assessed to feesnot paid on time.
Afee of$15.00 will be assessed on checks not honored bythe bank. Families on assistantprograms are required to make the weekly
payment each Mondayand the monthly reimbursement checks willbe forwarded to themuponreceipt.
FINAL NOTICE
Atwo week notice is required at the time oftermination.By signing a contract agreement, you agree to give a two week notice before
terminating care or paying the two week fee to terminate immediately.I would appreciate as muchnotice as possible, so ifyou give
more than two weeks, your child’s spot is guaranteed until your date of termination.I willnotask you toleavebecause ofa new child
coming in.
PARKING
I kindlyrequestthatifyou arrive at daycare between 7:15 and 7:45, you do not parkin front ofour garage.Mikeleaves for work
betweenthese times and a lot ofparents are coming and going so at times hegets blocked infor quite awhile.
