WHEREAS a controversy arose between (COMPANY NAME) and (CUSTOMER NAME)

and neither party admits responsibility nor assumes full liability, both parties, as evidenced by

their signature below, have agreed to the following terms to settle their dispute.

(CUSTOMER) has agreed to pay $(DOLLAR AMOUNT) as full and final settlement of all

claims due. Payment is due and payable upon receipt of this signed agreement.

Upon receipt and subsequent clearance of the agreed upon payment, both parties release each

other from any further claim or liability.
