REQUIREMENTS FOR COMMERCIAL DRIVERS

IMPORTANT: New Federal Regulations require submission of additional proofs which prohibit you

from renewing your Commercial Driver License (CDL) by MAIL. See the red text

below for additional information.

FOR ALL RENEWALS ON OR AFTER JUNE 18, 2015:

US Citizens and Lawful Permanent Residents:

- You must visit a DMV office and provide proof of your citizenship or unrestricted Lawful Permanent

Residency.

- One proof of residence is required each time you renew. If the address on your license renewal invitation

(MV-2) is correct, your MV-2 will serve as acceptable proof. Otherwise, you may present any of the

acceptable residency proofs listed on form ID-44CDL.

Applicants from a foreign jurisdiction:

- You must visit a DMV office and provide proof of your legal presence in the United States.

- One proof of residence is required each time you renew. If the address on your license renewal invitation

(MV-2) is correct, your MV-2 will serve as acceptable proof. Otherwise, you may present any of the

acceptable residency proofs listed on form ID-44CDL.

Note: Original documents must be provided at your DMV office.

Please refer to form ID-44CDL for a list of all the documents that you may submit as proof of US

Citizenship, Lawful Permanent Residency, foreign legal presence and New York residency.

The U.S. Department of Transportation (USDOT) Federal Motor Carrier Safety Administration regulations

establish minimum qualification standards for persons who operate (or expect to operate) commercial motor

vehicles, and establish requirements for state-issued commercial driver license documentation. The regulations

are found at 49 CFR 383.

In the past 10 years was a driver license issued to you from another state in the U.S. or the

District of Columbia?

¨Yes ¨No If ‘Yes’, clearly print the names of each one:

You MUST certify to DMV that you operate (or expect to operate) a commercial motor vehicle

(CMV) in one of the following four driving types (select ONLY one):

¨ Non-excepted Interstate (NI)- Certified medical status is required. You are age 21 or older and you

operate, or expect to operate, interstate (other than for excepted operation).

¨ Non-excepted Intrastate (NA)- Certified medical status is required. You are age 18 or older and you

operate, or expect to operate, in NYS only (other than for excepted operation).

¨ Excepted Interstate (EI)- You are age 18 or older and you operate, or expect to operate, interstate in

Excepted Operation ONLY. You must have A3 restriction.

¨ Excepted Intrastate (EA)- You are age 18 or older and you operate, or expect to operate, in

Excepted Operation ONLY and in NYS ONLY. You must have A3 and K restrictions.

If the driving type you selected requires certified medical status (NI or NA) you must provide a

legible copy of your current USDOT Medical Examiner’s Certificate to DMV when you renew, if it

is not already on file.

• Provide ALL information requested on this form. You MUST choose a driving type.

• Sign this form and print your name and driver license number in the spaces provided.

• Return this form in the self-addressed envelope OR, if you are required to renew in person, bring this form

with you to a Motor Vehicle office. In either case, include your completed license renewal application. Be

sure to follow all of the instructions on your license renewal application, including those for vision testing.

If certified medical status is required for the driving type you select on this form, with your renewal you

MUST
