I will be attending the 2015 AOCPMR Job Fair on Saturday, April 11 from 4:30-6:00pm in San Antonio, TX at the
Omni La Mansion.
I understand registration fee to participate is waived until December 1, 2014.
By signing this statement, I acknowledge and agree on behalf of myself and the company at
___________________________________________________________ (“Company”):
1. The American Osteopathic College of Physical Medicine and Rehabilitation (AOCPMR) is organizing the Job Fair for the
convenience of physicians, residents and osteopathic medical students.
2. The fact that physicians, residents and osteopathic medical students are permitted to participate in the Job Fair does not in
any way constitute endorsement of their qualifications and no advertisements or announcements implying such
endorsements will be permitted.
3. AOCPMR makes no representations or warranties concerning the Job Fair.
4. The Company agrees to hold harmless, and shall not seek remedy from the AOCPMR. The Company understands that the
AOCPMR has disclaimed all liability for damages, costs and expenses, including legal fees that may be incurred because of
its participation in the Job Fair, regardless of the cause, effects or fault.
