TRANSCRIPTS
If you are not degree-seeking, transcripts are not required.
First-Time-in-College Students – All first-time-in-college stu-
dents must submit final high school transcripts or equivalency
(showing actual graduation date). If you are a Florida public
high school graduate, Miami Dade College will electronically
request your records. However, you are ultimately responsible
for ensuring that the College receives your final high school
transcript or equivalency. Proof of high school graduation,
GED or approved equivalent is required prior to becoming eli-
gible for financial aid.
Foreign High School Graduates – You must submit original,
certified documents for proof of high school or equivalent
graduation. English translation may be required under certain
circumstances.
Transfer Students – If you are seeking a degree, you are re-
quired to provide official copies of academic records from each
college/university previously attended before evaluation for
credit can be considered. Your college/university transcripts
will be evaluated only if you are enrolled as a degree-seeking
student.
Note: Failure to submit all appropriate transcripts will jeo-
pardize future enrollment at the College and financial aid
eligibility.
ADVISEMENT
All new students must see an academic advisor before regis-
tering for their first term.
BACHELOR’S DEGREE APPLICANTS
The bachelor’s degree programs are limited-access programs,
and certain prerequisites must be completed before being
admitted. Admission to the College does not constitute admis-
sion to upper-division programs of study. A separate program
application is also required for each bachelor’s degree pro-
gram. Please consult the College Catalog on the Web at www.
mdc.edu.
NURSING AND ALLIED HEALTH
PROGRAM APPLICANTS
Admission to the College does not constitute acceptance into
any of the nursing or allied health programs. A separate pro-
gram application is also required. Please refer to the College

Notification of Social Security Number Collection and Usage by Miami Dade College
In compliance with Section 119.071(5), Florida Statutes, Miami Dade College (MDC) issues this notification regarding the purpose for the col-
lection and use of your Social Security Number (SSN). Miami Dade College collects and uses your SSN only to perform College duties and
responsibilities. To protect your identity, MDC will maintain the privacy of your SSN and never release it to unauthorized parties in compliance
with state and federal laws. The College assigns you a unique student/employee identification number which is used for associated employment
and educational purposes at MDC, including the access of your college records.
Miami Dade College may collect and/or use your Social Security Number for the following purposes:

Welcome to Miami Dade College. Please read the following instructions carefully. All information in this packet must be completed
before the application can be processed. Type or print legibly all information requested in blue or black ballpoint ink. All correspon-
dence will be mailed to the address you provide on the application.
A nonrefundable $30 application fee is required from all new college credit students. Fee is subject to change without notice. This fee
is charged only once, with this application. Please make your check or money order payable to Miami Dade college and write the last
four digits of your Social Security number on your payment. Mail your application to the campus you plan to attend at the address on
the next page. You may also submit the application in person.
admissions and Registration
• Federal legislation relating to the Hope Tax Credit requires that all postsecondary institutions
report student SSNs to the Internal Revenue Service (IRS). This IRS requirement makes it nec-
essary for MDC to collect the SSN of every student. a student may refuse to disclose his/her
SSn for this purpose, but he/she may be subject to iRS penalties.
• The Florida public school system uses the SSN as a student identifier. It is beneficial to
have access to the same information for purposes of tracking and assisting students in the
transition from one educational level to the next. The intent is to establish a comprehensive
management database of information which will co-reside with the Division of Public Schools
Information Database and the State University System Database to provide integrated infor-
mation at the state level for educational decision-making.
• SSNs appear on official transcripts and are used for business purposes in accordance with
parameters outlined by the U.S. Department of Education.
• Tracking uses are authorized by SBE Rule
6A-10955(3)(e); 1008.386, F.S. and the Gen-
eral Education Provisions Act (20 USC
1221(e-1)).
• Hope/Lifetime Tax Credit uses are autho-
rized by 26 USC 6050S and Federal Register,
June 16, 2000/IRC Section 25A

Florida Resident for Tuition Affidavit
A Florida “resident for tuition purposes” is a person who has, or a dependent person whose parents or legal guardian has, established and main-
tained a legal residency in Florida for at least twelve months. Residence in Florida must be as a bonafide domicile rather than for the purposes
of maintaining a residence incident to enrollment at an institution of higher education. To qualify as a Florida resident for tuition purposes, you
must be a U.S. Citizen, permanent resident alien, or legal alien granted indefinite stay by the U.S. Bureau of Citizenship and Immigration Services.
Other persons not meeting the twelve-month legal residence requirement may be classified as Florida residents for tuition purposes only if they
fall within one of the limited special categories authorized by the Florida Legislature. All other persons are ineligible for classification as a Florida
“resident for tuition purposes.” Living in or attending school in Florida will not, in itself, establish legal residence. Students who depend on out-
of-state parents for support are presumed to be legal residents of the same state as their parents.

Information on Florida Residency for Tuition Classification
Students who are admitted to public-supported postsecondary educational institutions in Florida must complete an affidavit and provide proof
of “Florida residency for tuition purposes” in accordance with state law.
1009.21, Florida Statutes (F.S.) and 6a-10.044, Florida administrative code (F.a.c.)
Non-U.S. citizen students and/or parents must provide evidence of eligible legal immigration status in the U.S. before being considered for
Florida resident fees. Please check with the Registrar’s Office for more specific information on the non-U.S. citizen categories that are eligible
to be considered for in-state fees. International Students and non-U.S. citizens admitted into the United States with the following visas are NOT
ELIGIBLE to pay Florida resident fees: B, C, D, F, H4 (if 21 or over), J, M, P and Q.

and at least two of the below-specified documents with dates that evidence the consecutive 12-month qualifying period. at least one of the

paycheck stubs from the previous 12 consecutive months of work in Florida and/or a letter on official company letterhead from
the employer verifying the previous 12 consecutive months of employment in Florida
• Proof of Homestead exemption in Florida
Second group (may be used in conjunction with one document from the first group)
• Declaration of domicile in Florida (12 months from the date the document was sworn and subscribed as noted by the
Clerk of Circuit Court)
• A Florida professional or occupational license
• Florida incorporation
• Documents evidencing family ties in Florida
• Proof of membership in Florida-based charitable or professional organizations
• Any other documentation that supports the student’s request for Florida resident status
Examples of “other” documentation from the second group:
• Utility bills and proof of 12 consecutive months of payments
• Lease agreement and proof of 12 consecutive months of payments
• Official Federal, State or court documents evidencing legal ties to Florida

For students in state custody, the Department of Juvenile Justice (DJJ) may facilitate residency determination by providing the college or univer-
sity with documentation evidencing parental or legal guardian residence in Florida for the qualifying period. The DJJ Address Verification Form
shall be considered acceptable documentation for residency determination in conjunction with additional information that demonstrates that the
parent or legal guardian has maintained legal residence in this state for at least 12 months prior to the first day of classes of the term for which
Florida residency-for-tuition-purposes classification is sought. Students enrolling in colleges or universities that have tuition and fees exempt
according to section 1009.25(2)(c), Florida Statutes, shall not be required to submit Florida residency documentation for tuition purposes.
