The following is a written agreement between Ed Altounian (THE DJ GUY)
and ________________________________________________________________, hereafter referred to as customer.

THE DJ GUY agrees to provide music services for customer on the ___________ da y of ________________, 20______.

Customer agrees to pay The DJ Guy for its services (make checks payable to Ed Altounian).
The balance of this contract is to be paid ten days prior to the event for all weddings unless otherwise instructed.
A minim um $100.00 deposit is required to sec ure services for any specified date (photo booth requires additional $100 deposit). The deposit is non-
refundable. Canc ellat i ons m ade less than 90 days prior to the event are subject to a cancell ation fee equal to 50% of the bal ance, 60 days 75% and 30
days 100%, l ess t he deposit. Changes to this polic y and/or refunds will be at the sole discretion of the THE DJ GUY. Instal lment payments are limit ed to
no more t han FOUR payments. Neither THE DJ GUY nor any of its associates shall be held li abl e for any loss or liability as a result of the services
performed. THE DJ GUY will not hold customer responsi bl e for any loss or liability, which may occur to the disc jockey or equipment, unles s c aused by
blatant or malicious wrong doing by cust om er or their guests. If the event is to be held out doors , custom er is responsibl e for providing adequate shelter
from the elements. The DJ assi gned to your event is based on availabilit y and pack age type. Standard wedding packages are excluded from t his and
your DJ will be assi gned at time of quote, prior to signi ng this contract. Photo booth attendant and/ or DJ reserve the right t o end services should guests
becom e malici ous or endanger t he well bei ng of any associate of T he DJ Guy.
