To be used with a valid Utah dealer plate. This application must be filled out completely and signed by both the authorized dealer

representative and the prospective buyer. It must also be notarized in the Prospective Buyer section.

Upon completion of the trip, the authorized dealer representative must sign and return the application for loaded demo permit within 10

days. Failure of the representative to meet this requirement is a class B misdemeanor.
