Certicate entitlement
This application form can only be used if the birth occurred in NSW. If
you are the person named on the certicate, or a parent of the person
named on the certicate, the birth certicate can be issued to you.
If you DO meet the above criteria, you must provide:

Please provide at least three forms of ID, one of each from
categories 1, 2 and 3. If you are unable to provide ID from
categories 1 and 2, you must still provide at least three forms of ID.

Send your certicate to a Passport Oce
The Registry provides a service to fax your certicate to an
Australian passport oce or other DFAT locations (Consulate,
Embassy or High Commission). Requests to send your certicate
information to other locations will not be actioned. Fee applies - see
www.bdm.nsw.gov.au for our current fees.

279mm x 355mm. They are beautifully presented, wrapped in
tissue paper and packed in a protective cylinder.
