How can I access online forms?
You can access the online form from the Registry’s website or go to:
onlineforms.bdm.nsw.gov.au
What can I do if I need help com pleting the online form?
If you hover over the fields on the form you will see some tool tips which will give you some
assistance in completing the form. There is also a link to the Help page from the online forms
homepage. If you are still having trouble completing the form call 137788.
What web browsers can I use to access the online form?
Internet Explorer 11, Chrome and Safari. The online form is also tablet and mobile friendly. If
your web browser is not sufficient you may receive the following message:
W arning: The browser version is out of date and the website may not work correctly. Please
upgrade your browser to obtain the best experience from this website.
What is an ORN?
An ORN is an Online Reference Number. Thisnumber will appear on the top right hand corner
once you have completed the first two sections of the form. The ORN can be used to resume an
application at a later time or you can quote the ORN if you need to contact the Registry about
your application.
Will I receive an email once my online form is saved or submitted?
You will receive a number of emails when using online forms. See below table for types of
emails you can expect to receive.
To ensure that emails regarding your online application are not sent to your email’s spam folder,
please add the following addresses to your email address book.

Can I apply for a certificate if I am not recorded on the registration?
If you are not recorded on the registration you must supply an authority letter and three form s of
identificationfrom a person who is authorised to apply for the certificate. All applications are
check by Registry staff to ensure you are entitled to the certificate. If you are not entitled to the
certificate, the Registry will be in contact with you to request further information.
If I haven’t submitted my online application, will it ex pire?
Online applications expire within 7 days. Once the online application expires, you will need to
start a new application.
Can I apply for a certificate if I live overseas or wish to post my certificate overseas?
Yes, you can enter an overseas address using online forms. The Countryfield defaul ts to
Australia. Select the country you are applying from or wish to send the certificate to from the
drop down list when completing the Residential or Postal Address section.
Can I apply for a commemorative certificate using online forms?
Yes, you can apply for commemorative birth or marriage certificates.
Can I use the online form if I require my certificate urgently? Is there a fee forthis
service?
Yes. In the Delivery and Payment section of the online form you can choose Yesfor Priority.
The application will be processed urgently and the certificate will be posted to you.
A service fee is incurred if you request your application to be process ed urg entl y . Refer to the
Regi stry’s w ebsi te for c ur r ent fees . Al ter na ti v ely, you w il l see the fees pay able on t he Delivery
and Payment screen as you progress through your online form .
How can I access my saved online form?
Refer to the Resume applicationand check your order statusguide on the Helppage for
further instructions.
Can I request for my certificate to be faxed to another department? Is there a fee for this
service?
The Registry can fax a copy of your certificate to an Australian Passport Office, Australian
Embassy or Australian Consulate if you provide the details in the Delivery and Payment
section o f your onli ne for m.

A service fee is incurred if you request yourcertificate to be faxed. Refer to the Registry’s
website for current fees. Alternatively, you will see the fees payable on the Delivery and
Paymentscreen as you progress through your online form.
How do I know which identification Document Number to enter?
On the Add Attachments screen you will see a Wher e is thi s ?link. Select the link and an
image will be displayed showing you which number to enter.
What types of attachments does the online form accept?
You may attach P DF , TIFF or JPEG fil es .
Can I collect my certificate if I use online forms?
No, your certificate will be sent to you by registered mail if you apply online.
I am trying to view a PDF version of my onli ne form. Why can’t I see it?
Your device settings may not allow pop ups. You will need to enable pop ups to see the PDF.
Can I track the progress of my online application?
Yes, you can do this by using the Check your order statusfeature. Refer to the Resume
applicationand check your order statusguide for furt her instr uct ions.
What if I don’t know the registration number?
You are not required to enter a registration to complete your application. If you do know the
registration number you can select Yesin theRegistration Number Known?field and
complete t he detai l s.
What if I don’t know the date of event?
In the Date of Event Knownfield, select No. Then enter a date range you wish the Registry to
conducta search for. The first 10 year search will be included in the cost of the certificate.
There will be an additional charge for subsequent years being requested per 10 year block.
Can I order more than one standard certificate?
Yes, you can order up to 9 standard certificates in one transaction.

Can I order more than one commemorative certificate?
Yes, you can order multiple commemorative designs. There is a limit of 9 commem orative
certificates per commemorative design.
Can I order different certificates in the one transaction?
No, you will need to apply for each certificate type separately.
Can I apply for a Family History certificate using online forms?
If you wish to apply for a family historyyou will need to use the following link:

What if I don’t know the parent’s nam es?
Enter Unknownin the nam e fields.
What if I forget my ORN?
If you have forgotten your password, refer to the Resume applicationand check your s ta tus

How do I know if my certificate has been posted to me?
You can go to Online Formsand Check Your Status . Your application status will be
Dispatchedand you will see the Postal Tracking Number(also known as registered m ail).
