The undersigned person has requested and has been granted permission to participate in Providence

In addition, the undersigned intends to ride with other persons in a vehicle owned by an individual and

not Providence College to activities associated with

In consideration for permission to participate in and to ride with other persons in a vehicle not owned by

Providence College to such activities, the undersigned, individually and on behalf of any heirs and

assigns does hereby:

1. WARRANT

that he/she chooses of his/her own volition to assume a risk of riding with others in a

vehicle not owned by Providence College;

2. WARRANT

that he/she is fully aware of any risk or danger inherent in riding with other persons

in a vehicle not owned by Providence College; and assumes all such risk of so doing to

himself/herself;

3. RELEASE, DISCHARGE, AND COVENANT NOT TO SUE

Providence College for any and all

claims and liability arising out of injury, death, or property damage resulting from the operation

of and/or being a passenger in any motor vehicle not owned by Providence College while

traveling to or from any activity associated with the Providence College

4. ACKOWLEDGE

that the undersigned is aware of the inherent danger of being a passenger in a

motor vehicle and will comply with all laws and regulations governing those activities,

ASSUMING ALL RISK

for himself/herself and all liability to others for failure to do so. No oral

representations or inducements have been made to obtain signatures on this agreement. If any

portion of this agreement is held invalid it is agreed that the balance thereof shall continue in full

legal force and effect.
