I certify all unexpired license plates and registration stickers were removed from each listed motor vehicle, the listed ownership evidence was
assigned to the named entity and is being surrendered to the department in accordance with Transportation Code, Chapter 501, the vehicles have
been permanently destroyed, and I understand these vehicles cannot be repaired, rebuilt, reconstructed, made operable, or retitled.

Surrendered Ownership Evidence for Vehicles Permanently Destroyed
Information
This form is for use by a licensed metal recycler only. Licensed salvage vehicle dealers must use the Surrendered
Ownership Evidence for Vehicles to be Dismantled, Scrapped, or Destroyed (Form VTR-340).
In accordance with Texas Administrative Code, Section 217.86, a metal recycler must submit this form, along with the
properly assigned ownership evidence for each listed motor vehicle, to a Texas Department of Motor Vehicles (TxDMV)
Regional Service Center before the 61st day
after the metal recycler permanently destroys the motor vehicle.
If a title document is the ownership evidence for the motor vehicle, the person or entity transferring the motor vehicle
must complete the assignment on the title as the “seller.” If the vehicle is subject to odometer disclosure, the seller must
also disclose the odometer reading, and the metal recycler must acknowledge the disclosure as the “purchaser.” Federal
law specifies a motor vehicle is subject to odometer disclosure when it is self-propelled, less than 10 years old, and has a
gross vehicle weight of 16,000 pounds or less.
Any license plates and registration stickers removed from the motor vehicles must be destroyed.
A metal recycler should keep, on the business premises, a record of each vehicle that is dismantled, scrapped, or destroyed,
and a photocopy of the front and back of the ownership evidence submitted to the department until the fourth anniversary
of the date that this form was submitted to the department.
Note: A vehicle reported on this form is considered a nonrepairable motor vehicle and cannot be repaired,
reconstructed, made operable, or retitled. The body and frame of a nonrepairable motor vehicle cannot be used to
assemble, build, construct, rebuild, or reconstruct another motor vehicle. The vehicle record will be notated accordingly.
